What Is Alarplasty?
Alarplasty — also called alar base reduction or nostril reduction surgery — is a focused cosmetic procedure that narrows the width of the nostrils by removing a small wedge of tissue from the alar base (the fleshy sides of the nostrils where they meet the cheek). The result is a more proportionate nostril width that complements the overall nasal and facial structure.
The procedure addresses several common concerns: nostrils that flare excessively (particularly during smiling), an alar base that is wider than the intercanthal distance (the space between the inner corners of the eyes — a common aesthetic proportion reference), or nostrils that appear round or overly open.
Alarplasty is one of the least invasive nasal procedures available. It is typically performed under local anaesthesia, takes one to three hours depending on technique, and requires no overnight hospital stay. Because the incisions are placed along the natural crease where the nostril meets the face, scarring is virtually invisible once healed.
This procedure is frequently performed as a standalone treatment but can also be combined with tip rhinoplasty or bridge augmentation for a more comprehensive nasal reshaping.
Who Is a Good Candidate?
You may be a good candidate if you:
- Have nostrils that are wider than you would like in proportion to your nose and face
- Experience excessive nostril flaring, especially when smiling or breathing deeply
- Want a more refined, narrower nostril appearance without altering other nasal structures
- Are at least 18 years old with completed facial growth
- Are a non-smoker or willing to stop smoking at least two weeks before and after surgery
Alarplasty may not be appropriate if you:
- Need changes to the nasal bridge, tip, or septum — a more comprehensive rhinoplasty approach may be necessary
- Have very thick alar skin, which may limit the degree of refinement
- Have active infections or skin conditions affecting the nasal area
- Have a history of keloid or hypertrophic scarring, which may affect incision site healing
How Alarplasty Is Performed
Alarplasty is a refined, precise procedure. Here is what happens during a typical nostril reduction surgery in Thailand.
Anaesthesia
Local anaesthesia is injected into the alar base area. You remain awake and comfortable throughout the procedure. Sedation can be added if you prefer.
Marking
The surgeon carefully marks the planned excision areas on both nostrils, ensuring symmetry and the correct amount of tissue removal. Precise marking is critical to achieving balanced, proportionate results.
Incision and tissue removal
A small wedge-shaped section of skin and soft tissue is excised from each alar base. The incision follows the natural crease where the nostril meets the cheek (alar-facial groove), which conceals the scar. In some cases, tissue is removed from the nostril sill (the floor of the nostril) or inside the nostril, depending on whether the width or the flare is the primary concern.
Closure
The incisions are closed with fine sutures, drawing the nostril edges closer together and narrowing the base. The surgeon checks symmetry from multiple angles before completing the closure.
Preparing for Your Alarplasty
Because alarplasty is minimally invasive, preparation is straightforward.
Medical Clearance
- • Provide your full medical history, medications, supplements, and allergy information.
- • Pre-operative blood tests and a brief physical assessment are completed before surgery.
Lifestyle Adjustments
- • Stop smoking at least two weeks before and after surgery to support wound healing.
- • Discontinue aspirin, ibuprofen, and blood-thinning supplements at least two weeks prior.
- • Avoid alcohol for at least 24 hours before the procedure.
Day of Surgery
- • Eat a light meal before arriving if local anaesthesia only is planned (your surgeon will advise).
- • Wash your face thoroughly; do not apply makeup.
- • Remove all jewellery and nose piercings.
Practical Planning
- • Arrange accommodation near your clinic.
- • Plan to stay in Thailand for a minimum of 9 days for suture removal and follow-up.
- • No travel companion is strictly necessary for this minor procedure, though having support is always helpful.
Recovery and Aftercare
Alarplasty recovery is one of the fastest among nasal procedures. Most patients experience minimal disruption to their daily routine.

Risks and Complications
Alarplasty is a low-risk procedure when performed by a qualified surgeon. However, all surgical procedures carry some degree of risk.
Common Side Effects
- • Mild swelling and tenderness — resolves within one to two weeks
- • Incision site redness — fades over several weeks to months
- • Temporary numbness — sensation returns as nerves heal
Less Common Complications
- • Infection — rare; minimised through sterile technique and wound care
- • Asymmetry — slight differences between nostrils; minor revision may be needed in rare cases
- • Visible scarring — uncommon when incisions are placed in the natural crease
- • Over-correction — nostrils narrowed too much, which can affect breathing
- • Under-correction — nostrils not narrow enough; a minor revision can address this
Choosing a surgeon with extensive alarplasty experience is the most important factor in achieving a safe, satisfying result.
