What Is a Brazilian Butt Lift?

A Brazilian butt lift is a body contouring procedure that enhances the shape, size, and projection of the buttocks. The term encompasses two distinct surgical approaches — fat transfer and gluteal implants — and your surgeon will recommend the best method based on your anatomy, goals, and available donor fat.

Fat Transfer BBL

The most popular approach. Fat is harvested from areas of the body where it is unwanted — typically the abdomen, flanks, thighs, or back — using liposuction. The fat is then purified and strategically injected into the buttocks to create fuller, rounder contours. This method offers the dual benefit of slimming the donor areas while augmenting the buttocks.

Gluteal Implants

For patients who lack sufficient body fat for a transfer, solid silicone gluteal implants provide an alternative path to buttock enhancement. The surgeon creates pockets either above or below the gluteus maximus muscle and positions the implants to achieve the desired shape and projection.

Combined Approach

For patients who want both structural projection and natural contouring, a surgeon may place implants for the core volume and use fat grafting around the implant edges and in the hip area for a seamless, natural result.

Buttock Lift

Patients with significant skin laxity — often following major weight loss — may benefit from a buttock lift, which removes excess skin and lifts the buttock tissue. This can be performed alone or in combination with augmentation.

Who Is a Good Candidate?

For a Fat Transfer BBL:

  • Adequate donor fat: You must have enough excess fat in areas such as the abdomen, flanks, back, or thighs to harvest a sufficient volume.
  • Good skin elasticity: Skin in the buttock area should have enough elasticity to accommodate the added volume smoothly.
  • Stable weight: Your weight should be stable for at least three months before surgery.
  • Non-smoker: Smoking severely impairs fat graft survival and wound healing.
  • BMI 23-30: Generally the ideal range for fat transfer.

For Buttock Implants:

  • Insufficient donor fat: Patients who are too lean for a fat transfer can achieve augmentation with implants.
  • Desire for significant projection: Implants can provide a degree of volume and projection that fat transfer alone may not achieve in slimmer patients.
  • Good general health: No uncontrolled medical conditions that could compromise healing.
  • Realistic expectations: Understanding the recovery process and implant maintenance.

How a BBL Is Performed

A buttock augmentation at a JCI-accredited hospital in Thailand typically follows this process:

Fat Transfer BBL

1
Anaesthesia: General anaesthesia is administered.
2
Liposuction: Fat is harvested from predetermined donor areas using tumescent liposuction. Common donor sites include the abdomen, flanks, lower back, and thighs.
3
Fat processing: The harvested fat is purified — centrifuged or filtered — to isolate healthy, viable fat cells and remove blood, oil, and damaged cells.
4
Fat injection: The purified fat is injected into the buttocks at multiple depths and locations using specialised cannulas. The surgeon sculpts the buttocks layer by layer.
5
Closure: Liposuction incision sites are closed with small sutures. A compression garment is applied to the donor areas and a specialised garment supports the buttocks.

Buttock Implant Procedure

1
Anaesthesia: General anaesthesia is administered.
2
Incisions: An incision is made in the natural crease where the buttock meets the upper thigh, or within the intergluteal cleft (between the buttocks).
3
Pocket creation: The surgeon creates a pocket either within the gluteus maximus muscle (intramuscular) or beneath it (submuscular), depending on anatomy and implant size.
4
Implant placement: The solid silicone implant is positioned within the pocket and adjusted for symmetry and natural appearance.
5
Closure: Incisions are closed in layers. A compression garment is applied.

Preparing for Your BBL

Careful preparation significantly improves outcomes and the recovery experience.

Medical Evaluation

  • • Comprehensive physical examination and medical history review.
  • • Pre-operative blood tests.
  • • Body measurements and photographs for surgical planning.

Lifestyle Adjustments

  • Stop smoking at least four weeks before and after surgery. Nicotine drastically reduces fat graft survival.
  • Discontinue aspirin, ibuprofen, and blood-thinning supplements at least two weeks prior.
  • Avoid alcohol for at least one week before surgery.

Weight Management

  • • Achieve and maintain a stable weight before surgery.
  • • For fat transfer BBLs, having some extra body fat in the donor areas can be beneficial.

Practical Planning

  • • Invest in a BBL pillow (a specialised cushion that keeps pressure off the buttocks while sitting).
  • • Prepare loose, comfortable clothing that does not compress the buttocks.
  • • Stock your recovery space with essentials within easy reach — you will need to minimise sitting and bending.

Recovery and Aftercare

Recovery after a BBL requires discipline, particularly regarding sitting and pressure on the buttocks.

First 48 Hours

Hospital Stay

You will remain in the hospital for two to four nights for monitoring. Pain medication is administered to manage discomfort. Drains may be placed at liposuction sites and are typically removed within one to two days. Swelling and bruising are significant in both the donor and buttock areas.
Week 1

Strict Positioning

Avoid sitting directly on your buttocks. Use a BBL pillow or lie on your side or stomach when resting. Compression garments must be worn as directed — typically on donor areas at all times. Walk short distances frequently to promote circulation and reduce clot risk. Sleep on your stomach or side.
Weeks 2–4

Gradual Improvement

Swelling and bruising gradually subside. You may begin sitting for short periods using a BBL pillow. Light activities can resume, but avoid any strenuous exercise. Follow-up appointments with your surgeon monitor healing progress.
Months 1–3

Resuming Activity

You can gradually return to normal sitting. Resume exercise cautiously after six to eight weeks, starting with low-impact activities. Some of the transferred fat (typically 20–40 percent) will be naturally absorbed by the body during this period — your surgeon accounts for this by slightly over-correcting during the injection phase.

Risks and Complications

Buttock augmentation is a safe procedure when performed by a qualified, experienced surgeon in an accredited facility. Risks include:

Common Side Effects

  • Fat absorption — a natural process in which the body reabsorbs a portion (typically 20–40 percent) of transferred fat. Surgeons compensate by injecting a calculated surplus.
  • Contour irregularities — lumps or uneven surfaces in the donor or buttock area, which often improve as swelling resolves.
  • Asymmetry — minor differences in shape or volume between sides may occur.

Less Common Complications

  • Infection — uncommon in accredited hospital settings.
  • Seroma or haematoma — fluid or blood collections that may require drainage.
  • Implant-related complications — capsular contracture, shifting, or discomfort (for implants only).
  • Fat embolism — a rare but serious risk specific to fat transfer. Substantially minimised when the surgeon injects fat above the muscle only and uses blunt-tipped cannulas.
