What Is Cheek Implant Surgery?

Cheek implant surgery — medically known as malar augmentation — is a facial contouring procedure that enhances the size, projection, and definition of the cheekbones. Solid, anatomically shaped medical-grade silicone implants are positioned over the existing malar (cheekbone) or submalar (below the cheekbone) region to restore or create the appearance of high, sculpted cheeks.

Well-defined cheekbones are one of the most universally recognised markers of facial attractiveness. They provide structural support to the midface, prevent the lower face from appearing heavy, and create aesthetically pleasing light-and-shadow contours. When cheekbones are naturally flat or volume has been lost through ageing, the face can appear gaunt, tired, or lacking definition.

Unlike injectable dermal fillers, which provide temporary volume lasting 12 to 18 months, cheek implants offer a permanent enhancement. The implants come in various shapes (malar, submalar, and combined) and sizes, allowing the surgeon to tailor the augmentation precisely to your facial anatomy. Results become fully visible approximately 3 months after surgery once all swelling has resolved.

Cheek augmentation is frequently combined with facelift surgery, chin augmentation, rhinoplasty, or fat grafting for comprehensive facial rejuvenation.

Who Is a Good Candidate?

You may be a good candidate if you:

  • Have flat, underdeveloped, or asymmetric cheekbones
  • Have lost midface volume due to ageing, creating a hollowed appearance
  • Want a permanent solution rather than repeated filler injections
  • Are in good overall health with no uncontrolled chronic conditions
  • Are a non-smoker or willing to stop smoking at least two weeks before and after surgery
  • Have fully developed facial bones (typically age 18 or older)
  • Have realistic expectations about outcomes

Cheek implants may not be appropriate if you:

  • Have active dental infections or oral disease
  • Have very thin soft tissue that may make implant edges visible
  • Have significant facial asymmetry caused by skeletal differences requiring orthognathic surgery
  • Prefer a temporary or adjustable approach — dermal fillers or fat grafting may be better suited

How Cheek Implant Surgery Is Performed

1

Anaesthesia

The procedure is performed under general anaesthesia or local anaesthesia with sedation, depending on surgeon preference and whether additional procedures are being performed.

2

Incision

A small incision (approximately 2 cm) is made inside the upper mouth, between the cheek and the upper gum. This intraoral approach ensures there is no visible external scar.

3

Pocket Creation

The surgeon carefully dissects a pocket directly over the malar bone, lifting the soft tissue to create a space that matches the size and contour of the selected implant.

4

Implant Placement

Pre-selected, anatomically shaped silicone implants are positioned symmetrically over each cheekbone. The surgeon may secure the implants with a small titanium screw or rely on the precision of the pocket.

5

Symmetry Verification

The surgeon confirms that both implants are evenly placed by sitting the patient upright and comparing each side. Fine adjustments are made before closure.

6

Closure

The incisions are closed with absorbable sutures inside the mouth. Gentle compression dressings or tape may be applied. The operation typically takes approximately 1 hour.

Preparing for Your Cheek Implant Surgery

Medical Clearance

  • • Share your full medical history, medications, supplements, and allergies with your surgeon.
  • • Blood tests and a physical examination are completed before surgery.
  • • Dental health is assessed, as the intraoral approach requires a clean surgical field.

Lifestyle Adjustments

  • Stop smoking at least two weeks before and after surgery.
  • Discontinue aspirin, ibuprofen, and blood-thinning supplements at least two weeks prior.
  • Avoid alcohol for at least 24 hours before surgery.

Day of Surgery

  • • No food or drink for at least 6 hours before the operation if general anaesthesia is used.
  • • Remove all jewellery, piercings, and contact lenses.
  • • Wear loose, front-opening clothing.

Practical Planning

  • • Arrange accommodation close to the hospital.
  • • Plan to stay in Thailand for a minimum of 7 days.
  • • Stock up on soft foods — a liquid or soft diet is recommended for the first few days.

Recovery and Aftercare

Days 1–3

Early Recovery

Moderate swelling of the midface and cheeks is expected. Mild to moderate discomfort is well-controlled with prescribed oral medication. A soft or liquid diet is necessary. Sleep with the head elevated at 30 to 45 degrees. Antiseptic mouthwash is used after meals.
Days 4–7

Follow-Up

Swelling begins to subside noticeably. Intraoral sutures dissolve on their own within 7 to 14 days. A follow-up appointment confirms the implants are correctly positioned. Most patients are cleared to fly home after 7 days.
Weeks 2–4

Progressive Healing

Residual swelling and mild stiffness gradually resolve. You may return to a normal diet as comfort permits. Light exercise can resume around week 2. Avoid contact sports for at least 6 weeks.
Months 1–3

Final Results

The cheeks' final shape and projection become fully apparent approximately 3 months after surgery. Any temporary numbness of the upper lip or cheek area typically resolves within this timeframe.

Risks and Complications

Common Side Effects

  • Swelling and bruising — most prominent in the first week, resolves over 2 to 4 weeks
  • Temporary numbness — upper lip or cheek sensation may be altered for several weeks
  • Stiffness or tightness — the cheek area feels firm as tissues adapt

Less Common Complications

  • Infection — minimised through sterile technique and prophylactic antibiotics
  • Implant displacement or asymmetry — rare with an experienced surgeon
  • Nerve injury — branches of the infraorbital nerve can be stretched
  • Bone erosion — long-term pressure can cause minor resorption
  • Capsular contracture — uncommon with solid silicone
