What Is Chin Reduction?

Chin reduction — also referred to as reduction mentoplasty, chin shaving, or chin contouring — is a facial bone-sculpting procedure that reduces the size, projection, or width of an overly prominent chin. Unlike chin augmentation, which adds volume, reduction mentoplasty removes or reshapes bone to bring the chin into better proportion with the rest of the face.

An overly large chin can disrupt facial balance in several ways: it may make the midface appear flat, the forehead appear small, or give the lower face a heavy, masculine appearance. For patients undergoing facial feminisation, chin reduction is one of the most impactful procedures for softening the lower facial contour.

The procedure is performed entirely through an incision inside the lower lip (intraoral approach), leaving no visible scarring on the face. The surgeon uses precision instruments — including oscillating saws and burrs — to sculpt the chin bone to the desired shape and size. In some cases, a sliding osteotomy technique is used to physically reposition a segment of bone rather than simply shaving it.

Chin reduction is frequently combined with jaw reduction, rhinoplasty, or other facial contouring procedures for patients seeking comprehensive facial harmonisation.

Who Is a Good Candidate?

You may be a good candidate if you:

  • Have a chin that appears disproportionately large, long, or wide relative to your other facial features
  • Feel your chin creates an overly angular, square, or masculine lower face
  • Are seeking facial feminisation and wish to soften the chin contour
  • Are in good overall health with no uncontrolled chronic conditions
  • Are a non-smoker or willing to stop smoking at least two weeks before and after surgery
  • Have fully developed facial bones (typically age 18 or older) and realistic expectations

Chin reduction may not be appropriate if you:

  • Have active infections, cysts, or untreated dental disease in the chin or jaw area
  • Have significant bite or skeletal misalignment that requires orthodontic or orthognathic intervention first
  • Are on medications that impair bone healing (such as bisphosphonates) without medical clearance
  • Have unrealistic expectations about the degree of change achievable through bone contouring alone

How Chin Reduction Is Performed

The operation typically takes approximately 2 hours. Patients spend 1 night in hospital for monitoring.

1

Anaesthesia

The procedure is performed under general anaesthesia. A qualified anaesthesiologist monitors your vitals throughout.

2

Incision

A small incision is made inside the mouth, along the lower gum line between the lip and the gum. This intraoral approach ensures there is no visible external scar.

3

Bone Exposure

The soft tissue is carefully elevated to expose the chin bone (mentum) and the relevant portions of the lower jaw border.

4

Bone Contouring

Using an oscillating bone saw and rotary burrs, the surgeon sculpts the chin to the pre-planned dimensions. Depending on the patient's anatomy, the surgeon may shave the bone, narrow the lateral borders, or shorten the chin using a horizontal osteotomy that removes a segment of bone and re-fixates the lower fragment with titanium plates and screws.

5

Symmetry Check

The surgeon verifies symmetry by visual inspection and palpation, making fine adjustments as needed before closing.

6

Closure

The incision is closed with absorbable sutures. A compression chin strap is applied to minimise swelling and support the newly contoured bone during early healing.

Preparing for Your Chin Reduction

Medical Clearance

  • • Share your full medical history, medications, supplements, and allergies.
  • • Blood tests, a physical examination, and facial imaging (X-ray or CT scan) are completed before surgery to map the bone structure.
  • • Dental health is assessed to ensure no active infection is present near the surgical site.

Lifestyle Adjustments

  • Stop smoking at least two weeks before and after surgery.
  • Discontinue aspirin, ibuprofen, and blood-thinning supplements at least two weeks prior.
  • Avoid alcohol for at least 24 hours before surgery.

Day of Surgery

  • • No food or drink for at least 6 hours before the operation.
  • • Remove all jewellery, piercings, and contact lenses.
  • • Wear loose, front-opening clothing.

Practical Planning

  • • Arrange accommodation close to the hospital for your post-discharge recovery.
  • • Plan to stay in Thailand for a minimum of 7 days.
  • • Prepare soft foods and nutritional shakes — a liquid and soft diet is essential for the first week.

Recovery and Aftercare

Days 1–2

Hospital Stay

You remain in hospital for 1 night. Moderate to significant swelling, tightness, and bruising are expected. Pain is managed with medication. A compression chin strap is worn continuously. Oral hygiene is maintained with an antiseptic mouthwash.
Days 3–7

Early Recovery

Swelling peaks around day 3 and then begins to subside. A liquid or very soft diet continues. Numbness of the lower lip and chin is common. Follow-up appointments confirm proper healing. Most patients are cleared to fly home after 7 days.
Weeks 2–4

Progressive Healing

Swelling continues to diminish. You can gradually transition back to a normal diet. Avoid hard, crunchy, or chewy foods for at least 3 weeks. Light exercise may resume around week 3, but avoid activities that risk impact to the chin.
Months 1–6

Final Results

Residual swelling can take 3 to 6 months to resolve completely. The final chin contour becomes fully apparent once all deep swelling has subsided. Numbness typically resolves within 3 to 6 months.

Risks and Complications

Common Side Effects

  • Swelling and bruising — peaks around day 3 and resolves over several weeks
  • Temporary numbness — sensation returns within 3 to 6 months in most patients
  • Jaw stiffness — difficulty opening the mouth wide, resolving within 2 to 4 weeks

Less Common Complications

  • Infection — minimised through sterile technique and oral hygiene
  • Asymmetry — significant asymmetry is rare with an experienced surgeon
  • Nerve injury — permanent numbness of the lower lip or chin is uncommon
  • Bone irregularities — palpable or visible irregularities may occasionally require revision
  • Hardware-related issues — if titanium plates are used, loosening is rare but possible

Choosing a board-certified surgeon at a JCI-accredited facility significantly reduces these risks.
