What Is a Forehead Lift?

A forehead lift — also called a brow lift or browplasty — is a facial rejuvenation procedure that elevates drooping brows, smooths horizontal forehead creases, and reduces the vertical frown lines (glabellar lines) between the eyebrows. The procedure repositions the underlying muscles and tissues of the forehead to restore a naturally youthful, relaxed expression.

As we age, the soft tissues of the forehead descend under the influence of gravity, sun exposure, and repetitive muscle movement. This causes the brows to sag — particularly at the outer edges — and deepens the horizontal and vertical lines across the forehead. A drooping brow can also contribute to upper eyelid heaviness, making brow lift and blepharoplasty a frequently combined procedure.

Modern forehead lifts in Thailand are most commonly performed using an endoscope-assisted technique, which uses small incisions hidden within the hairline and a tiny camera to guide the procedure. This minimally invasive approach results in less scarring, reduced swelling, and faster recovery compared to the traditional open (coronal) brow lift.

The procedure typically takes two to four hours under general anaesthesia and requires a short hospital stay of two to three nights.

Who Is a Good Candidate?

You may be a good candidate if you:

How a Forehead Lift Is Performed

Understanding the surgical process helps set realistic expectations. Here is an overview of a typical endoscopic brow lift in Thailand.

Step 1 — Anaesthesia

The procedure is performed under general anaesthesia with a qualified anaesthesiologist monitoring your vitals throughout.

Step 2 — Incisions

Three to five small incisions (each approximately 1–2 cm) are made behind the hairline. An endoscope — a thin tube with a camera — is inserted through one incision to provide the surgeon with a magnified view of the underlying structures on a monitor.

Step 3 — Tissue Release

The surgeon carefully releases the forehead skin and tissue from the underlying bone, allowing the brow and forehead to be repositioned. The corrugator and procerus muscles (responsible for frown lines) are weakened or partially removed to smooth the glabellar area.

Step 4 — Brow Elevation and Fixation

The brow and forehead tissue are lifted to a more youthful position and secured using bioabsorbable fixation devices, sutures, or small screws anchored to the skull bone. These hold the tissue in place while healing occurs and are absorbed by the body over time.

Step 5 — Closure

The small incisions are closed with sutures or clips. A light dressing or bandage is applied around the forehead.

The operation typically takes 2 to 4 hours. Patients stay in hospital for 2 to 3 nights for monitoring and initial recovery.

Preparing for Your Forehead Lift

Medical Clearance

  • • Share your full medical history, including medications, supplements, allergies, and any previous facial surgery.
  • • Pre-operative blood tests, a physical examination, and potentially an ECG are completed before surgery.
  • • Discuss your hairline concerns with your surgeon — endoscopic techniques generally do not alter hairline position.

Lifestyle Adjustments

  • Stop smoking at least two weeks before and after surgery — smoking impairs wound healing and increases complications.
  • Discontinue blood thinners (aspirin, ibuprofen, fish oil, vitamin E) at least two weeks prior.
  • Avoid alcohol for at least 24 hours before surgery.

Day of Surgery

  • • No food or drink for at least 6 hours before the operation.
  • • Wash your hair the evening before or morning of surgery.
  • • Remove all jewellery, glasses, and contact lenses.
  • • Wear loose, front-opening clothing.

Practical Planning

  • • Arrange accommodation close to the hospital for your post-discharge recovery.
  • • Plan to stay in Thailand for a minimum of 14 days.
  • • Have a companion travel with you for assistance during the first few days after discharge.

Recovery and Aftercare

Forehead lift recovery requires patience, but most patients find it more comfortable than expected — particularly with the endoscopic approach.

Days 1–3: Hospital Stay

You remain in hospital for 2 to 3 nights. Expect swelling across the forehead and around the eyes, as well as bruising that may extend to the upper eyelids and cheeks. Head elevation is essential. Pain is moderate and well-controlled.

Days 4–7: Early Recovery

Once discharged, continue resting with your head elevated. Swelling peaks around day 3 to 4 and then progressively improves. Bruising begins to turn yellow-green and fade. Avoid bending forward, heavy lifting, and strenuous activity.

Weeks 1–2: Transition

Sutures or clips are removed around day 10 to 14. Most social bruising and swelling have resolved. You may feel numbness or tingling across the forehead and scalp — this is normal and resolves over weeks to months.

Weeks 3–6: Progressive Improvement

Swelling continues to decrease and the lifted brow position stabilises. You can return to desk work around week 3. Avoid vigorous exercise until cleared by your surgeon (typically around week 4 to 6).

Months 2–6: Final Results

Residual swelling and numbness resolve. Incision scars within the hairline become virtually invisible. The final, refreshed brow position and smooth forehead contour are apparent within 3 to 6 months. Results typically last 7 to 10 years or longer.
