What Is a Breast Lift?

A breast lift — medically known as mastopexy — is a surgical procedure that raises and reshapes sagging breasts. The surgeon removes excess skin, tightens the surrounding tissue, and repositions the nipple and areola to a higher, more forward-facing position on the breast mound.

Breast ptosis (sagging) is a natural consequence of ageing, hormonal changes, pregnancy, breastfeeding, and gravity. Over time, the skin loses elasticity, the breast tissue descends, and the nipple points downward rather than forward. While a well-fitted bra provides temporary support, only surgery can permanently restore the breast's position and shape.

A breast lift does not significantly change the size of the breasts. Patients who wish to increase volume typically combine mastopexy with breast implants (augmentation mastopexy), while those with excessively large breasts may benefit from combining a lift with breast reduction. Important Note: Nipple sensation may be temporarily altered following mastopexy and can take up to 12 months to fully return. The ability to breastfeed may also be affected, though many women retain this ability after a lift.

Who Is a Good Candidate?

You may be a good candidate if you:

  • Have breasts that have lost shape, volume, or firmness
  • Have nipples and areolae that point downward or fall below the breast crease
  • Have one breast that sits noticeably lower than the other
  • Have stretched skin and enlarged areolae
  • Are at or near a stable weight — significant weight changes after surgery can affect results
  • Are a non-smoker or willing to stop smoking at least two weeks before and after surgery
  • Are in good overall health with no uncontrolled chronic conditions
  • Have realistic expectations about scarring, outcomes, and recovery

A breast lift may not be appropriate if you:

  • Plan future pregnancies — pregnancy and breastfeeding can reverse the results of mastopexy
  • Are currently breastfeeding or have recently stopped
  • Have a BMI significantly above the healthy range, as this increases surgical risks
  • Smoke and are unwilling to quit for the required period
  • Have active breast disease or abnormalities that require evaluation first

How a Breast Lift Is Performed

Understanding the surgical process helps set realistic expectations. Here is a step-by-step overview of mastopexy in Thailand.

Anaesthesia

The procedure is performed under general anaesthesia with a qualified anaesthesiologist monitoring your vitals throughout.

Incision pattern

The incision pattern depends on the degree of ptosis and the amount of excess skin to be removed. The three main patterns are:

  • Periareolar (donut lift) — a circular incision around the areola only; suited for mild ptosis with minimal excess skin.
  • Vertical (lollipop lift) — an incision around the areola and vertically down to the breast crease; suited for moderate ptosis.
  • Inverted-T (anchor lift) — an incision around the areola, vertically down to the crease, and along the inframammary fold; suited for significant ptosis or when a large amount of skin must be removed.

Tissue reshaping

The surgeon lifts and reshapes the underlying breast tissue to create a firmer, more rounded breast mound. Internal sutures may be used to support the new breast shape from within.

Nipple repositioning

The nipple-areola complex is moved to a higher, more youthful position on the reshaped breast mound. If the areolae are enlarged, they can be reduced in diameter during this step.

Excess skin removal

Stretched, redundant skin is excised from below and around the areola. The remaining skin is tightened and re-draped over the newly shaped breast.

Implant placement (if applicable)

If augmentation mastopexy is being performed, breast implants are positioned — typically submuscular or dual-plane — before the skin closure.

Closure

Incisions are closed in layers with absorbable and non-absorbable sutures. Surgical drains may be placed for the first 24 to 48 hours. A supportive surgical bra is applied.

The operation typically takes 4 to 6 hours, depending on the technique and whether implants are included. Patients spend 2 to 3 nights in hospital.

Preparing for Your Breast Lift

Proper preparation is essential for a safe procedure and smooth recovery.

Medical Clearance

  • • Share your full medical history, medications, supplements, and allergies with your surgeon.
  • • Blood tests and a physical examination are completed before surgery.
  • • A recent mammogram or breast ultrasound may be required for patients over 40 or those with a family history of breast cancer.

Lifestyle Adjustments

  • Stop smoking at least two weeks before and after surgery — smoking significantly impairs wound healing and increases the risk of nipple and skin necrosis.
  • Discontinue aspirin, ibuprofen, and blood-thinning supplements (fish oil, vitamin E, ginkgo biloba) at least two weeks prior.
  • Avoid alcohol for at least 24 hours before surgery.
  • Maintain a stable weight in the weeks leading up to surgery.

Day of Surgery

  • • No food or drink for at least 6 hours before the operation.
  • • Shower on the morning of surgery; do not apply lotions, deodorant, or perfumes to the chest area.
  • • Remove all jewellery, piercings, and contact lenses.
  • • Wear loose, front-opening clothing — you will not be able to raise your arms easily for several days.

Practical Planning

  • • Arrange accommodation close to the hospital for your post-discharge recovery.
  • • Plan to stay in Thailand for a minimum of 12 days.
  • • Purchase two or three soft, front-closure sports bras (no underwire) in your expected post-operative size.
  • • If possible, have a companion travel with you for assistance during the first few days.

Recovery and Aftercare

Breast lift recovery follows a predictable timeline, though individual experiences vary based on the technique used and whether implants are included.

Days 1–3

Hospital Stay

You remain in hospital for 2 to 3 nights. Moderate pain, swelling, and tightness across the chest are managed with medication. Drains (if placed) are typically removed within 24 to 48 hours. A supportive surgical bra is worn continuously. Early walking is encouraged to promote circulation. Arms should remain below shoulder height.
Days 4–7

Early Recovery

Bruising and swelling begin to subside. Incision sites are cleaned and assessed at follow-up appointments. Continue wearing the supportive bra at all times except when showering. Sleep on your back with the upper body slightly elevated.
Weeks 1–2

Transition

Sutures are removed around day 7 to 10. Light daily activities resume, but avoid lifting anything heavier than 2 kg. Most patients can fly home after 12 days with surgeon clearance. The breasts will appear high and swollen — this is normal and will settle over the coming weeks.
Weeks 3–6

Progressive Recovery

Swelling continues to decrease and the breasts begin to "drop and fluff" into a more natural shape and position. Return to desk work around week 3 to 4. Light exercise (walking, lower body) may resume with surgeon approval. Avoid upper body workouts, heavy lifting, and underwire bras for at least 6 weeks.
Months 2–12

Final Healing

The breasts continue to soften and settle into their final shape over 3 to 6 months. Scars progress from pink or red to pale lines over 12 to 18 months — silicone sheets and sun protection help optimise scar appearance. Nipple sensation, if altered, typically returns within 6 to 12 months, though some changes may be permanent.

Risks and Complications

Like all significant surgeries, mastopexy carries potential risks. Choosing a board-certified surgeon at a JCI-accredited facility significantly reduces these risks.

Common Side Effects

  • Swelling and bruising — resolves over several weeks
  • Temporary altered nipple sensation — numbness or hypersensitivity typically resolves within 6 to 12 months
  • Scarring — permanent but fades significantly over 12 to 18 months; placement depends on the incision pattern used

Less Common Complications

  • Infection — minimised through sterile environments and prophylactic antibiotics
  • Bleeding or haematoma — drains reduce this risk; significant bleeding may require intervention
  • Wound-healing issues — more likely in smokers and diabetic patients; nipple or skin loss is rare
  • Asymmetry — minor differences between breasts are normal; significant asymmetry may require revision
  • Loss of nipple sensation — permanent loss is uncommon but possible
  • Changes to breastfeeding ability — many women retain the ability to breastfeed, but it cannot be guaranteed
  • Deep vein thrombosis — prevented through early mobilisation and compression stockings
